Mandibular Advancement Devices for Obstructive Sleep Apnea: A Systematic Review and Meta-Analysis

Abstract

Background: This systematic review and meta-analysis aimed to evaluate the effectiveness of mandibular advancement devices (MADs) in treating Obstructive Sleep Apnea (OSA), focusing on the impact of age and body mass index (BMI) on the apnea-hypopnea index (AHI) and minimum oxygen saturation (min SpO2). Methods: A literature search was conducted in PubMed and Google Scholar for studies published from January 2013 to January 2023. Two independent authors screened the studies, assessed and evaluated risk of bias. Disagreements were resolved by consensus or consultation with a third reviewer and senior experts. The initial search yielded 1110 references, with 13 selected for inclusion. Results: MADs significantly reduced AHI and increased min SpO2 in OSA patients. The mean difference in AHI was 15.75 (95% CI 13.19–18.30, p <0.00001). Minimum SpO2 improved post-treatment (-3.70, 95% CI-5.52 to-1.88, p < 0.00001, Z = 3.98). Conclusion: This meta-analysis demonstrated that MADs effectively reduced AHI severity and increased min SpO2 in OSA patients. While BMI had no significant effect, age had a notable impact on treatment outcomes.

Bruxism: Innovative Perspectives in Assessment and Therapy

Abstract

Bruxism is a prevalent parafunctional behavior marked by involuntary clenching, grinding, bracing, or thrusting of the mandible during wakefulness or sleep, whose cumulative mechanical load can surpass the adaptive capacity of dental and musculoskeletal tissues and precipitate tooth wear, orofacial pain, and temporomandibular disorders. Reported prevalence estimates range 3.5–96%, reflecting heterogeneity in diagnostic definitions, assessment methods, and study populations, with adult rates averaging approximately 7% and pediatric estimates spanning 3.5–40.6%. Etiologically, bruxism emerges from the interplay of peripheral factors—such as facial morphology and occlusal discrepancies—central neuromuscular dysregulation, psychosocial stressors, pharmacological influences, and genetic predisposition. An integrated diagnostic schema—categorizing cases as possible, probable, or definite based on self-report, clinical examination, electromyography, and polysomnography— enhances consistency across settings. Management prioritizes reversible measures, such as patient education, behavioral modification, physical therapy, pharmacotherapy, and occlusal splint therapy, to reduce parafunctional load, with occlusal adjustment, botulinum toxin injections, or surgical intervention reserved for refractory presentations.

Current Perspectives in Treatment of Obstructive Sleep Apnea with Mandibular Advancement Devices: A Narrative Review

Abstract

Purpose of Review This narrative review intends to provide a thorough analysis of the effectiveness, and comparison of the mono-block vs duo-block Mandibular Advancement Devices (MAD) and the potential side effects of these devices in managing Obstructive Sleep Apnea (OSA).

Recent Findings MAD therapy is a widely accepted non-invasive treatment option for individuals with OSA, who may not accept positive airway pressure treatment. Both appliances are effective in reducing AHI, however, mono-block MAD is considered to have better short-term outcomes and higher patient tolerance compared to duo-block MAD. While MADs are generally well-tolerated, they can be associated with side effects. Patient education on proper device care and adjustment is crucial for minimizing adverse effects.

Summary Careful patient selection, thorough monitoring, and addressing potential adverse effects are essential for optimizing treatment outcomes. Future research should focus on refining patient selection criteria, and assessing the long-term effectiveness of MADs in diverse populations.

Bu çalıştay raporunun amacı diş hekimlerine bruksizmin güncel tanımı, bruksizm tipleri, etiyolojisi, tanısı, çok boyutlu değerlendirme kriterlerinin belirtilmesi ve yönetimi konusunda literatür destekli bir rehber sunmaktır. Böylece rutin dental muayene sırasında bruksizmin, risk faktörlerinin ve sebeplerinin ayrıştırılması ve farkındalığın oluşması sağlanacaktır.

Uyku ile İlişkili Solunum Bozukluklarının Protetik Diş Tedavisi Açısından Değerlendirilmesi: Çalıştay Raporu

Uyku ile ilişkili solunum bozukluğu olarak tanımlanan basit horlama ve obstrüktif uyku apne (OSA) tanı ve tedavisi tıbbi branşlar tarafından yapılmakla birlikte, son yıllarda multidisipliner yaklaşım gerektiren bu sendromun tedavisinde diş hekimlerinin ve tedavilerinin etkinliği oldukça önem kazanmıştır. Bu amaçla diş hekimlerinin lisans ve lisans üstü eğitimlerine basit horlama ve OSA tanı ve tedavisi konularının dahil edilmesiyle, hastaların farkındalıklarının artırılması ve varsa hastalığın tanısının konulmasında yardımcı olmaları ve yönlendirmeleri ile ilgili yeterli ve uygun eğitimi alması amaçlanmaktadır. Böylece rutin dental muayene sırasında uyku bozukluğuna neden olabilecek risk faktörlerinin ve sebeplerinin ayrıştırılması ve farkındalığının oluşması sağlanacaktır.

Dental muayene sırasında risk faktörleri ve semptomları açısından hasta değerlendirildiğinde, risk faktörleri bulunan hastanın uykuda solunum ya da horlama ile ilgili bir şikayeti olmasa da diş hekimi tarafından hastada bir farkındalık yaratılmalıdır. Bu diş hekiminin teşhis ve yönlendirmedeki sorumluluğudur. Diğer sorumluluğu ise, teşhisi konulan ve bir ağız içi aparey tedavisi uygun görülen hastaların tedavisine katkı sunmaktır.

Bu çalıştay raporunun amacı, diş hekimlerine, bu konu hakkındaki eğitimlerine yön verecek katkı sağlamak ve teşhisten tedaviye güncel literatür destekli bir rehber sunmaktır.
